To enroll, simply print the enrollment application, complete, and mail to:

Enrollment Notes: 

If your complete enrollment form and payment are received by the 20th of the current month, you will be eligible on the 1st of the next month.  If received after the 20th, you will be eligible on the 1st of the following month.

You will need to indicate your choice of dentist on the enrollment form.  Select a dentist from the Directory of Participating Dentists.
